Social Security number and date of birth must be provided for all dependents. Failure to submit the dependent\u2019s Social Security number will result in termination\/denial of coverage (exceptions: newborns age six months or less).<\/strong><\/p>\n\n\n\n

Documentation also applies to life insurance coverage.<\/p>\n\n\n\n

No documentation is required at Open Enrollment to delete a dependent.<\/p>\n\n\n\n

All documentation should contain the employee\u2019s name and Social Security number.<\/p>\n<\/div>\n\n\n\n
