These are the top five chronic diseases prevalent in our population. Your medical plan carrier may reach out to you, offering support if you are coping with any of these chronic diseases.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"accordion-block\"><button class=\"wp-block-wps-content-blocks-accordion accordion-button\" title=\"Toggle Accordion\" aria-expanded=\"false\" aria-controls=\"accordion-content_274\"><span class=\"accordion-title\" id=\"accordion-title_983\">Incentive Program<\/span><span class=\"accordion-icon\"><\/span><\/button><div class=\"accordion-content\" id=\"accordion-content_274\" aria-labelledby=\"accordion-title_983\" role=\"region\">\n<p>Anthem BCBS POS Blue Open Access and Kaiser HMO non-Medicare retirees only can earn a $150 incentive for completing an annual physical examination with their Primary Care Physician. The incentive must be earned by December 31, 2025, and redeemed and rewarded by March 31, 2026.<\/p>\n\n\n\n<p>Anthem members can read more about the incentives <a href=\"https:\/\/benefits.atlantaga.gov\/2025\/document\/anthem-wellness-program-retirees\/\" data-type=\"document\" data-id=\"1478\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>.<\/p>\n\n\n\n<p>If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact the DHR \u2013 Benefits and we will work with you (and, if you wish, with your doctor) to find a wellness program with the same reward that is right for you in light of your health status.<\/p>\n\n\n\n<p><strong>Anthem members<\/strong> also can download the <a href=\"https:\/\/www.sydneyhealth.com\/\" target=\"_blank\" rel=\"noreferrer noopener\">Sydney Health app<\/a>. Log into the app using the same username and password you use for <a href=\"http:\/\/www.anthem.com\" target=\"_blank\" rel=\"noreferrer noopener\">www.anthem.com<\/a>.&nbsp;<\/p>\n\n\n\n<p>From the app, you can access rewards: press MENU from your home screen then select ACCESS CARE. Go to the My Health Dashboard, and click My Rewards. You will see the incentive activities all in one place. View Featured programs to find your My Health Check-in (heath assessment) and more! <\/p>\n<\/div><\/div>\n\n\n\n<div class=\"accordion-block\"><button class=\"wp-block-wps-content-blocks-accordion accordion-button\" title=\"Toggle Accordion\" aria-expanded=\"false\" aria-controls=\"accordion-content_613\"><span class=\"accordion-title\" id=\"accordion-title_414\">Vendor-Sponsored Wellness Programs<\/span><span class=\"accordion-icon\"><\/span><\/button><div class=\"accordion-content\" id=\"accordion-content_613\" aria-labelledby=\"accordion-title_414\" role=\"region\">\n<p>The Medicare Advantage plan carriers also provide wellness programs and local fitness centers in some locations. Please check your plan website for program information.<\/p>\n\n\n\n<p>UnitedHealthcare Medicare Advantage members can stay active with a free gym membership. You can also take advantage of a personalized fitness plan, online brain health programs from AARP Staying Sharp, and Fitbit workouts you can do at home or locally with other Silver Sneakers members.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"accordion-block\"><button class=\"wp-block-wps-content-blocks-accordion accordion-button\" title=\"Toggle Accordion\" aria-expanded=\"false\" aria-controls=\"accordion-content_8\"><span class=\"accordion-title\" id=\"accordion-title_878\">COA Employee Wellness Center<\/span><span class=\"accordion-icon\"><\/span><\/button><div class=\"accordion-content\" id=\"accordion-content_8\" aria-labelledby=\"accordion-title_878\" role=\"region\">\n<p>City of Atlanta pre-age 65 retirees are eligible to use the City\u2019s onsite health clinic. You must present a proper Medical Plan ID card.<\/p>\n\n\n\n<p>Free medical and pharmacy services will be available to retirees only (not to dependents of retirees) who are enrolled in the non-Medicare plan options.
