The assessment will assist you in determining which activity will suit your healthcare needs.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Kaiser members:&nbsp;<a href=\"http:\/\/www.kp.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">www.kp.org<\/a><\/li>\n\n\n\n<li>Anthem BCBS members: Go to <a href=\"http:\/\/www.anthem.com\" target=\"_blank\" rel=\"noreferrer noopener\">www.anthem.com<\/a>, select My Health Dashboard, then Programs, My Rewards and much more!<\/li>\n<\/ul>\n\n\n\n<p>Anthem members also can download the <a href=\"https:\/\/www.sydneyhealth.com\/\" target=\"_blank\" rel=\"noreferrer noopener\">Sydney Health app<\/a>. Log into the app using the same username and password you use for <a href=\"http:\/\/www.anthem.com\" target=\"_blank\" rel=\"noreferrer noopener\">www.anthem.com<\/a>.&nbsp;<\/p>\n\n\n\n<p>From the app, you can access rewards: press MENU from your home screen then select ACCESS CARE. Go to the My Health Dashboard, and click My Rewards. You will see the incentive activities all in one place. View Featured programs to find your My Health Check-in (heath assessment) and more! <\/p>\n<\/div><\/div>\n\n\n\n<div class=\"accordion-block\"><button class=\"wp-block-wps-content-blocks-accordion accordion-button\" title=\"Toggle Accordion\" aria-expanded=\"false\" aria-controls=\"accordion-content_224\"><span class=\"accordion-title\" id=\"accordion-title_424\">Disease Management<\/span><span class=\"accordion-icon\"><\/span><\/button><div class=\"accordion-content\" id=\"accordion-content_224\" aria-labelledby=\"accordion-title_424\" role=\"region\">\n<p>Contracted insurance vendors manage chronic diseases such as diabetes, heart disease, coronary artery disease (including circulatory restrictions and strokes), musculoskeletal disorders (including lower back pain), and digestive disorders. These are the top five chronic diseases prevalent in our population. Your Medical Plan carrier may reach out to you, offering support if you are coping with any of these chronic diseases.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"accordion-block\"><button class=\"wp-block-wps-content-blocks-accordion accordion-button\" title=\"Toggle Accordion\" aria-expanded=\"false\" aria-controls=\"accordion-content_258\"><span class=\"accordion-title\" id=\"accordion-title_651\">Incentive Program<\/span><span class=\"accordion-icon\"><\/span><\/button><div class=\"accordion-content\" id=\"accordion-content_258\" aria-labelledby=\"accordion-title_651\" role=\"region\">\n<p>Active employees can earn wellness incentives at the end of the plan year.<\/p>\n\n\n\n<p>Anthem BCBS offers<strong> <\/strong>$250 for completing a <a href=\"https:\/\/benefits.atlantaga.gov\/2025\/document\/anthem-wellness-program\/\" data-type=\"document\" data-id=\"495\">variety of screenings<\/a>. <\/p>\n\n\n\n<p>Kaiser offers $300 for completing all of the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Total health assessment<\/li>\n\n\n\n<li>Biometric screening<\/li>\n\n\n\n<li>Cancer screenings<\/li>\n<\/ul>\n\n\n\n<p>All 2025 incentive amounts must be earned, redeemed and rewarded by December 31, 2025. Incentives will be delivered as a gift card.<\/p>\n\n\n\n<p>If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means.
